Medical services available through this platform are provided solely by independently licensed healthcare providers. Each provider is responsible for the privacy practices related to the care they deliver. Direct Providers Care, LLC provides administrative, operational, and technology services in support of telehealth delivery and does not practice medicine.

Your PHI may be used by your healthcare provider to provide, coordinate, or manage your medical care through telehealth services. This may include communication with other healthcare professionals involved in your care, referrals, or care coordination when clinically appropriate.
Your PHI may be used to facilitate payment for services, including processing membership payments, billing services, and coordinating benefits when applicable. This may include disclosures to payment processors or third parties involved in payment administration.
We may use and disclose PHI for operational purposes, including quality assurance, system administration, compliance activities, auditing, training, customer support, and improving the services offered through the platform.

Telehealth services involve the electronic transmission of medical information. While reasonable safeguards are used to protect your information, no system is entirely risk-free. By using this platform, you acknowledge and consent to electronic communications related to your care.
Providers may determine that telehealth is not appropriate for certain conditions and may recommend in-person care, testing, or referral when clinically necessary.
We implement administrative, technical, and physical safeguards designed to protect PHI from unauthorized access, use, or disclosure. Access to PHI is limited to authorized personnel and providers as necessary to perform their functions.
In the event of a breach of unsecured PHI, we will notify affected individuals in accordance with applicable law, including the nature of the breach, information involved, and steps you can take to protect yourself.
We reserve the right to change this Notice at any time. Changes will apply to all PHI we maintain. The revised Notice will be posted on our website and will include a new effective date.
